Supplimental Nutrition Assistance Program (SNAP)

Program Approval Letter, Benefit Statement, or Verification of Coverage Letter, should contain the following basic information:

  1. Name of the program,
  2. Name of the beneficiary,
  3. Eligibility dates, OR
  4. Current participation status

Federal Public Housing Assistance

Public Housing Assistance (FPHA)

FPHA award letter, should contain the following basic information:

  1. Name of the program,
  2. Date of the award,
  3. Name of the beneficiary, and
  4. Award amount.
Veterans Benefits

Federal Veterans Affairs (VA) Veterans Pension or Survivors Pension

Pension Grant Letter, should contain the following basic information:

  1. Participant’s name
  2. Address
  3. A decision about the participant’s monthly entitlement amount
  4. Payment start date
TANF

Tribally-Administered Temporary Assistance to Needy Families (TTANF)

TATANF Award letters, should contain the following basic information:

  1. Name of the program,
  2. Name of the beneficiary,
  3. Address of the beneficiary, and
  4. Date of the award
FDPIR

Food Distribution Program on Indian Reservations (FDPIR)

Notice of Action (award letter) acknowledging eligibility for FDPIR benefits;

  1. Name of the beneficiary;
  2. The beginning and ending dates of the award or certification period; and
  3. The telephone number of the Food Distribution Program office, and the name and address of the person to contact for additional information.

OR

FDPIR participation documents (e.g., a benefit card or copy of a benefit card);

  1. Name of the beneficiary;
  2. The beginning and ending dates of the award or certification period; and
  3. The telephone number of the Food Distribution Program office, and the name and address of the person to contact for additional information.

Providing Proof Of Income

If your total household income is at or below 135% of the poverty guidelines set by your state and/or the federal government, you may qualify for Lifeline service. To receive your Lifeline discount based on total household income, you must provide copies of one of the documents listed below.

Acceptable Proof of Income

  • Prior Year’s State, Federal or Tribal Tax Return
  • Social Security Benefits Statement
  • Retirement/Pension Benefits Statement
  • Divorce Decree or Child Support Documents
  • Veterans Administration Benefits Statement
  • Current Income Statement from Employer or Paycheck Stubs
  • Unemployment/Workers Compensation Benefits Statement
  • You may qualify for Lifeline if your income is 135% or less than the Federal Poverty Guidelines

If you provide documentation that does not cover a full year (such as current paycheck stubs), you must submit three (3) consecutive months’ worth of the same type of document from the previous twelve months.

Providing Proof Of Identification

In addition to providing proof of qualifying programs, you will also need to provide an accepted form of identification.

Acceptable Identification Documentation

  • Driver's license or Passport (unexpired)
  • Birth certificate
  • Certificate of Naturalization or of U.S. Citizenship
  • Permanent resident or resident alien card (unexpired)
  • U.S. government, military, state, or Tribal issued ID (unexpired)
  • Military discharge documentation
  • Weapons permit (unexpired)
  • Government assistance program document (that includes proof of identity)
  • Unemployment or worker's compensation statement of benefits
  • Statement of benefits from a qualifying program (that includes proof of identity)

Lifeline Program

Due to sharing many of the same eligibility requirements those currently enrolled in the lifeline program may qualify for The Affordable Connectivity Program (ACP) also.
