Reverifying Your Lifeline Service in Missouri
The National Verifier has officially launched in Missouri and you are now required to reverify your Lifeline eligibility by 1/6/2020. This means you will need to provide your eligibility requirements — or risk being disconnected from your Lifeline account.
Subscribers will receive a letter in the mail directly from the Lifeline Administrator (USAC) requesting eligibility reverification. To continue receiving Lifeline service, please complete the form and send it to:

You may contact Lifeline consumer support to get your reverification status by calling (800) 234-9473 Monday through Sunday, 9:00 AM to 9:00 PM ET (press 1 for English or press 2 for Spanish).
The Lifeline Administrator recently launched the “National Verifier” eligibility tool in Missouri, requiring reverification of all Lifeline subscribers.
Regardless of whether you have recently recertified with Assist Wireless, you will need to complete the required forms and submit them to the Lifeline Administrator or risk losing your Lifeline discount.
If you have not yet recertified this year, you will not be required to recertify again. (If you have already recertified with Assist Wireless this year, see FAQ#2.)
The Lifeline administrator will send you a letter outlining specific requests to submit proof of eligibility such as:
Simply submit photocopies or pictures of all the requested items listed in your unique letter. Copies or pictures can be made by a scanner, copy machine, camera, or smartphone.
After you have completed the form, return the copies to the Lifeline Administrator (USAC) by mail at: USAC Lifeline Support Center, P.O. Box 7081 London, KY 40742. Be sure to include the cover sheet with the unique ID and barcode, and copies of all required documentation.

If you do not have access to a printer, we recommend you try your local public library (ID may be required), or forward the form to a local print center, such as an Office Max, Staples, or UPS Store (call locations for specific directions on how to submit for printing services).

If you have moved since your last Lifeline eligibility recertification (or initial application, if with Assist for less than 1 year), you will need to make sure your address is updated with Assist Wireless. You can update your address here or call 855-392-7747.
Only one Lifeline discount is permitted per household, so failure to update your address could result in disconnection of your Lifeline service.
